Evaluation Survey

The City of Kingston is looking for feedback on the new Jumpstart Inclusive Playground at Shannon Park. Your responses will help us understand the impact this project has had on community members and also help us design better playgrounds in the future. Survey responses will also help provide information needed to complete final reporting requirements for some of the grants that funded the playground.

All responses are anonymous. The survey should take 5 minutes to complete.

Thank you for your feedback!
